Thank you for your interest in becoming a South Carolina Department of Disabilities and Special Needs (SC DDSN) qualified service provider. The SC DDSN provider application process is now OPEN. Prospective providers may apply at any time by responding to the Application for Provider Qualification.
SC DDSN is seeking providers to be qualified to deliver services to people with Intellectual Disabilities/Related Disabilities (ID/RD), Autism, and Head & Spinal Cord Injuries/Similar Disabilities (HASCI). Service providers are needed in all areas of the State to adequately serve people who are eligible for and authorized to receive these services. Submitted applications should be thorough, complete, well organized, and tabbed. When submitting, applications should be in Portable Document Format (“PDF”) and may be submitted via e-mail to providerapplications@ddsn.sc.gov. If desired and requested, SC DDSN can provide a link to securely exchange files. Requests for secure exchange should be made via email to bsimpson@ddsn.sc.gov.
Please note, providers already qualified by SC DDSN are not required to re-apply. However, a provider already qualified by SC DDSN must apply if they desire to deliver a new service. With questions concerning the application, please contact Bill Simpson at 803-898-9614 or bsimpson@ddsn.sc.gov.
